An investigation found no equipment problems in the death of a City of Columbia temporary worker.

Summer employee 23-year-old William Bell died July 8 when he got caught between the cab and bed of a dump truck. It was Bell's fifth year working with the City of Columbia's Parks and Recreation Department.

City officials hired an outside firm to investigate bell's death.  Their report says the truck was working properly and there were no problems that could have contributed to the incident.
